Some article of clothing that cannot be removed is responsible for the changes. Would be good for in-the-shower-but-wearing-clothes captions or impractical or inappropriate outfits for the context situation they're in. Personal bonus points for thongs, bikinis, or anything super girly (ribbons, frills, pink, sequins or glitter). Maybe bonus points too for people recognizing that they've been wearing the same piece of clothing every time they've seen them.
Or, for actual lactation, maybe someone transformed and now opts (presumably out of noble obligation but secretly for pleasure) to help feed the poor.
Or a desperate entrepreneur in a high-profile TG case now makes commercials trying to sell their milk, and even though the public ridicule is constant and unrelenting, the numbers don't lie, he's actually selling pretty well.
Or the back pain/immobility of an extreme change leads one to accept the offer from a clearly-obsessed (though trying to hide it) friend to offer to carry them around everywhere they go.
Or someone uses the transformed's (relative) immobility as an opportunity to exploit, by snapping photos for a porn site without consent. The exploiter is making bank and lives comfortably, the transformed is not.
Personally I've also got a thing for dark-skinned women, and would love to see more involving them.
